#AI reads Urine# Alterations of urinary proteomic profiles are correlated with return to sport after anterior cruciate ligament reconstruction

Published 08 March, 2026

This study focuses on issues related to patients' return to sport (RTS) after anterior cruciate ligament reconstruction (ACLR). Thirty patients who were at least 9 months postoperatively were recruited and divided into the RTS group and the non-RTS group. Through clinical evaluations and urinary proteomic analysis, significant differences in knee muscle characteristics (such as limb circumference and isokinetic muscle strength) were found between the two groups. A total of 3,433 urinary proteins were identified, among which 20 were upregulated and 58 were downregulated in the RTS group, involving key pathways including regulation of the actin cytoskeleton, calcium ion binding, and the IL-17 signaling pathway. Using bioinformatics and machine learning methods, Glypican-3 (GPC3), Phosphatidylinositol transfer protein alpha isoform (PIPNA), Rap guanine nucleotide exchange factor 3 (RPGF3), Histone H1.5 (H15), and Small proline-rich protein 3 (SPRR3) were identified as potential biomarkers, which are associated with muscle recovery and functional performance. The study indicates that muscle function is the main factor affecting RTS after ACLR, and these biomarkers can provide references for monitoring rehabilitation progress, formulating personalized intervention plans, and helping to improve patients' RTS outcomes.
